The following forms are available (in pdf format) for download from our site for your convenience.  They can be filled out and then either dropped off, faxed or mailed to our office.

Currently, our Water Service Agreement form is not available on-line.  To establish a new water service at your property (either Residential or Non-Residential), you are going to have to come into our main office, and fill out this form in person. 

In addition to the Water Service Agreement, Customers will also be provided an informational letter concerning New Construction and New Tap Purchases as they pertain to the District's Backflow Prevention Policies.
